Outreach support and Floating support

Outreach Support

Outreach Support, where accommodation is provided and support is offered with housing.

To access this service a referral is required from either CHART (Crosby Housing Association Reablement Team) or an individual’s Community Psychiatric Nurse (CPN)

Floating Support

Floating support, is support provided to an individual within their own home.

This service is accessed by contacting the manager listed below and requesting a Floating Support Referral form.

How we can help

Both Outreach and Floating Support help people tackle all kinds of issues; from rent arrears and benefits issues to activities that reduce social isolation. This support enables people to sustain their tenancies, find the services they need and build a better life for themselves within their community

Support is provided to individuals and support hours vary according to each person’s requirements, on the outcome of assessment, or recommendation from an individual’s community psychiatric nurse (CPN).

The staff team comprises of support workers who work 9-5 Monday to Friday.


Sefton Outreach service is based in North and South Sefton.
