Thriving is a ‘super’ social prescribing service for people often excluded from existing services as a result of their mental illness. The service offers a bespoke assessment and support package to enable people to build up their local social/community networks.

Our advisors look at a range of social determinants of wellbeing, including housing, finance, isolation, activity, employment, and volunteering and, by supporting the person to identify their own priorities, develop a plan to move things forward over an agreed time period

Service Aims

The service aims to support people identified to live better, more meaningful lives within their own community. We will achieve this with the provision of social based interventions. If we achieve these aims, CMHTs will also see a reduction in over use of services, such as calling emergency services and attending A&E.

Our Team

We have 3 Community Support Officers who between them have over 25 years frontline experience across a range of services all mainly working with people in the community. Their role is to support people in a person centred way to grow their network of activities and services to both support and improve their mental health while helping them to identify their own needs and priorities specifically around the wider determinants of wellbeing – finances, housing, employment, activity, volunteering and isolation. All staff enjoy the challenge each day brings, as no 2 days are the same. Staff like watching people develop, grow and widen their skills they then see how people’s confidence and self-esteem builds which then influences an improvement in both their mental and physical health.

Hours of Operation

Support is provided Monday to Friday, 9am to 5pm.

Eligibility Criteria and Referral Route

The service is available to residents of Liverpool, Sefton and Kirkby. All people referred must be age 18+ on a CMHT caseload.
