Based at our new project in Brierfield supporting complex young people in transition to live in a scheme of bungalows. The role will include time on rota and time off rota to support the Service Manager to deliver the service and support the team.
Senior Transforming Care Practitioners take a leading role within the staff team, to provide support to young persons in transition. The individuals that we support have complex needs including learning disability, mental health problems, physical health and diminished capacity.
There is a requirement for on-call on a rota basis, for which an additional £30 per day is paid.
Sleep in shifts are also paid in addition to salary.
