Shift Pattern: Monday – Friday 9am-5pm

The role

This is an exciting floating support worker role, offering the opportunity to work across our supported living day services assisting the staff teams in the day-to-day care and support. Taking the lead on coordinating activities and events at services, using initiative and being proactive in providing fun and exciting opportunities for those living within our services.The role will also include covering outreach visits supporting individuals in their own homes and out in the community. You’ll experience a diverse and rewarding role where flexibility, creativity, and compassion are key.

As a Floating Support Worker, you will:

  • Promote health, wellbeing, and independence for the people you support
  • Provide meaningful practical and emotional support tailored to individual needs
  • Support people to build skills, confidence, and strong community connections
  • Work across day services and outreach settings, bringing energy and continuity to the teams
  • Plan, organise, and support engaging activities and events within services
  • Encourage and promote social opportunities that reduce isolation and enrich lives
  • Empower individuals to achieve their personal goals in line with their agreed support plans

This dual role of working within supported living services as well as the community outreach service will provide flexible opportunities for teamwork and working independently. When working independently, you’ll never feel alone. You’ll receive guidance and check-ins from, Senior Support Worker and Service Manager, alongside ongoing support from your colleagues. You’ll also connect with the wider outreach and day services teams through quarterly team meetings, ensuring collaboration, shared learning, and a strong sense of team spirit.

Job Description:

The main duties of the role are to:

Under the guidance of the Senior Support Worker and Service Manager

  • Be allocated an advanced rota of consisting of support visits and day services shifts.
  • Take part in helping people draw up their person-centred plan and follow the agreed action plan
  • Work in partnership with others to allow independence of choice
  • To act with integrity and to identify opportunities to celebrate achievements to allow a positive, respectful and trusting relationship to grow
  • Support people to improve their confidence and self esteem
  • Support people to manage their own physical, mental health and wellbeing in a positive manner and to promote a healthy lifestyle and improving self-care
  • Support people to meet their leisure, work, social, emotional and spiritual needs
  • Support people to access the community and taking part in community activities
  • Support people to manage finances to meet domestic responsibilities and personal aspirations
  • Provide advice and support with domestic tasks and practical issues such as dealing with neighbours and keeping their house clean
  • Support people to be the best versions of themselves and promote a good quality of life
  • Support people who may have alcohol and/or drug issues
  • Support people to maintain the safety and security of their home
  • To prompt individuals and undertake personal care duties as required.
  • To write up and record activities undertaken
  • To report any significant change in an individual’s health, behaviour or wider situation to a manager and liaise with clinical teams.
